The NHS and local councils have come together in 44 areas covering all of England to develop proposals and make improvements to health and care. These proposals, called sustainability and transformation plans (STPs), are place-based and built around the needs of the local population.

This page contains the national information regarding the STP that you may find useful.

NHS England STP Animation

NHS England recently published an online animation explaining how and why STP proposals are trying to improve health and care for patients.

You can now watch the animation below:

They have also published a blog post here by Dr Arvind Madan, NHS England’s Director of Primary Care, discussing the benefits of bringing care closer to people’s homes. Further materials will be published over the coming weeks, including more blog posts, some case studies setting out what individual STPs might mean for patients, and infographics reinforcing the main points of the animation.

It is hoped that this will assist local STP areas to build understanding and support for the rationale and potential benefits of STPs among staff, patients and other important groups.
